You will also receive a notification email to your inbox confirming that the request has been sent successfully. If you do not receive a confirmation, please check your Junk or Spam email folders before contacting the Practice.


Our joint injection request form is encrypted and secured by a 128bit SSL certificate, and sends your request information in the content of an email. Although this is sent ONLY to the Practice, we are unable to guarantee complete privacy during the transmission.
